Here is the tuition schedule for 2012-2013.

Below are the forms that must be filled out to succesfully register your children for the 2012-2013 school year at CHDS. Enrollment forms were due April 3rd. If you have not done so, please take care of this as soon as possible after Pesach. There will be a late fee of $100.00 per child.

• Application for admission (for new students)
• Re-registration material (for returning students)
• Physician's report
• Emergency illness form
• Emergency medical form
• Non prescription medical form
• CHDS Grandparents form
• Pick up permission form
• Field trip permission slip
• Internet permission slip
• Introductory letter and information for FACTS grant and aid applications (must be completed no later than May 2 for all those applying for financial aid).
